Three levels of transition planning
We identify three levels of transition planning, based on a young person's needs:
Level 1 - Universal transition planning
For all young people, including those with no additional support needs. This covers the support and guidance all young people need to plan for their future.
Level 2 - Transition planning for young people with additional support needs
For young people who need specific guided support to reach a positive post-school destination - for example, those with a mild autistic spectrum disorder or those requiring additional assessment arrangements such as a scribe, extra time or digital papers.
Level 3 - Transition planning for young people with significant and complex needs
For young people who need a high level of support and multi-agency planning - for example, those with moderate or severe learning difficulties, physical disabilities, autistic spectrum disorder or emotional and behavioural difficulties. For looked after children, careful consideration must be given to whether Level 3 support is needed.
How transition planning works
Transition planning begins well before a young person leaves school and involves the young person, their family and all the agencies that support them. Our guidance sets out clear procedures for each level of planning, including timescales, who should be involved and what documentation is needed.
