Help on Leaving hospital

Introduction

All people admitted to hospital are assessed as to whether extra support is needed on discharge and if so a referral is made to Community Care for assessment.  there are several options for service provision that can be used to met assessed needs for individual people on discharge.

Rapid Response

Working jointly with NHS Orkney, to provide home care support to vulnerable people in order to prevent unnecessary admissions to hospital or residential care and where possible to facilitate early discharge.

Night Care Services

Providing support for a service user who is terminally ill, either to prevent hospital admission or enable hospital discharge to facilitate an individual’s wish to die in their own home.

Sleep-in Services

Providing support for older and disabled people who require occasional help during the night. The service supports the carer who normally assists the service user overnight.

Community Care Alarms

Providing Community Care Alarms for people who, because of age or disability, may need to summon help and who have been assessed as requiring a community care alarm. This service is provided by Age Concern Orkney on behalf of the Council

Home Support services

Providing help for people in supported housing or who are tenants and need extra help with living independently. This service complements any care service which may also be provided.
